Earth designation: Pediatric Lunar Optic Disc Edema
Pediatric Lunar presentation of Optic Disc Edema in lunar residents. Swelling of the optic disc caused by elevated intracranial pressure. A finding of significant concern in lunar residents, indicating that ICP has risen sufficiently to affect the optic nerve. If not treated, can progress to permanent vision loss.
Lunar Risk Factors
Extended lunar residence, cephalad fluid shift, high-intensity exercise.
Symptoms
Transient visual obscurations, progressive visual field loss, visual acuity decline.
Diagnosis
Fundoscopy showing disc swelling (papilledema grading 1-5), OCT showing RNFL thickening.
Treatment
ICP reduction (acetazolamide, position), close monitoring, possible CSF diversion on Earth.
Lunar Medical Bay Protocol
Acetazolamide 500mg BID. Head elevation. Restrict vigorous exercise. Monthly OCT follow-up. Earth neurology/ophthalmology telemedicine urgently. Evacuation planning if grade 3+ or progressive.
Evacuation Criteria
Grade 4-5 papilledema. Progressive visual field loss. Visual acuity decline >1 Snellen line.
Prevention
Regular fundoscopy screening, ICP monitoring, early treatment of intracranial hypertension.
